Sassy Paws Pet Styles LCC, hereafter referred to as "SPPS", has my permission to do whatever it feels is appropriate for the well-being of my pet. Should SPPS determine that veterinary care is warranted, I agree to pay all associated fees and costs. If my pet's veterinarian is not readily accessible, SPPS is authorized to use a veterinarian of its choice. SPPS will not be held responsible for any conditions or problems discovered/uncovered during the grooming process. Should veterinary care be necessary as a result of any services rendered by SPPS, SPPS must be notified within 24 hours of such care. Reimbursement for such care will be at the sole discretion of SPPS. We will not be responsible for any sickness or injury cause by the pet to itself during the grooming process. I warrant that my pet is current with the vaccinations required by law, and that my pet is healthy to the best of my knowledge.

SPPS will not be held responsible for clipper burn, minor nicks, or skin-irritation resulting from grooming; nor will SPPS be held responsible for stressful effects that grooming may have upon a geriatric pet.

SPPS shall not be held liable for any after-grooming effects of de-matting clipping procedures or problems uncovered on badly matted or otherwise neglected coats including, but not limited to: itchiness, skin redness or self-inflicted irritations/abrasions from excessive external rubbing. I understand that time and costs associated with de-matting are unpredictable and subject to the particular condition of my pet; consequently, I agree to pay whatever fees are incurred as a result of de-matting. Whether the pet is "matted", furthermore, is at the sole determination of SPPS.

SPPS DOES NOT ACCEPT PET WITH FLEAS. If fleas are found on your pet, it is policy of SPPS to have you pick up your pet IMMEDIATELY. If someone is unreachable it is SPPS policy to give a flea bath without needing to speak to an owner prior to performing this service. There is an extra charge for this service. Flea shampoo kills the fleas ON your pet. If fleas are in your home or your pet's area, the flea shampoo will NOT protect your pet from them once they leave our salon. Also, in the case of a tick infestation SPPS reserves the right to perform a tick bath if two or more ticks are found on your pet at the pet owner's expense.

SPPS reserves the right to refuse to groom any pet for the health and safety of the stylist and the animal. A soft muzzle and or other restraints may be used in attempt to perform services. Services may be discontinued or refused if SPPS determines that the animal presents an unsafe condition. Such conditions include, but are not limited to: behavioral issues, health problems, parasites, etc.

I, as the pet owner, understand that SPPS is a cage free environment for friendly well behaved, altered (spayed or neutered) pets. I have been honest in my assessment of my pet's behavior towards other animals and people. SPPS reserves the right to refuse to allow pets to be social with other pets in the salon if they exhibit any aggressive behavior such as dominating, growling, snapping, or biting, etc. I understand as the pet owner that I will be held responsible for any and all costs of damage to property or other pets due to my pet's actions. **If you are not comfortable with your pet being free to roam the salon with other pets and people, please let SPPS know and we will kennel your pet where it can relax and have its own space in the salon. I agree to inform SPPS prior to grooming if the pet has ever bitten any human or other animal, or if the pet has any aggressive tendencies whatsoever. I understand and agree that I will be held solely liable for any harm, injury, or property damage caused by my pet. **

I understand that the missed appointments will incur a fee. The first missed appointment will incur a 30% fee of the estimated grooming cost, the second a 40% fee, the third a 50% fee. To avoid this fee all appointments must be canceled 24 hours in advance. (Emergencies will be taken into consideration on an individual basis)

I understand that if I am over 15 minutes late for my appointment that I may be required to re-book and missed appointment fees may apply.

I agree to pick up my pet at the arranged time. If I am unable to pick-up by the agreed time when I am called to pick up my pet I will notify the groomer when I am available. I further agree to pay the late pick up fee of $1.00 per minute for every minute late (for the first 60 minutes) after the close of the salon. I understand if my pet is here longer than 60 minutes past closing, I will be charged $50.00 for every additional 30 minutes.

I affirm that I am the rightful legal owner of the pet for which services are rendered. I agree to pay all costs for the grooming of my pet including, but not limited to: special handling requirements, de-matting, and fees due for the late drop-off, pick up, or missed appointments.
